Testing Algorithms for Mycobacterium tuberculosis Complex using Real Time- Polymerase Chain Reaction: When to implement GeneXpert assay?
نویسندگان
چکیده
Study Design: Traditionally, Mycobacterium tuberculosis complex (MTB-complex) is identified by conventional methods. However, molecular methods are now being used more often. The previous algorithm of acid fast bacilli (AFB) smear positive patients who were new to the Bureau of Clinical Laboratories (BCL) was run on the GeneXpert automatically. The Xpert® MTB/RIF assay, performed on the GeneXpert® Instrument Systems, is a qualitative, nested real-time polymerase chain reaction (PCR) in vitro diagnostic test for the detection of MTB-complex DNA in raw sputum or concentrated sputum sediment prepared from induced or expectorated sputum. In specimens where MTB-complex is detected, the Xpert MTB/RIF assay also detects the rifampinresistance associated mutations of the rpoB gene. This assay was used to test 551 tuberculosis specimens between January 8, 2015 and October 12, 2016 using the new algorithm that is based on AFB smear results in addition to physician requests. If a patient is AFB smear positive and is not on file at the BCL, PCR by GeneXpert is performed. If a patient is smear negative and a physician requests PCR by GeneXpert based on clinical presentation of the patient, PCR is also performed.

Comparison of a real-time polymerase chain reaction-based system and Erlich-Ziehl-Neelsen method with culture in the identification of Mycobacterium tuberculosis.
BASCKGROUND/AIM Mycobacterium tuberculosis is still a major health problem throughout the world, especially in developing countries. Disease control heavily depends on the establishment of early diagnosis. The aim of this study is to compare the efficacy of culture, GeneXpert MTB/RIF device, and Erlich-Ziehl-Neelsen direct microscopic method. MATERIALS AND METHODS A total of 927 samples (243 ...

In 2012, Uganda introduced the use of GeneXpert MTB/RIF (Cepheid, Sunnyvale CA), a sensitive, automated, real-time polymerase chain reaction-based platform for tuberculosis (TB) diagnosis, for programmatic use among children, adults with presumptive human immunodeficiency virus (HIV)-associated TB, and symptomatic persons at risk for rifampicin (RIF)-resistant TB. The effect of using the platfo...

We investigated the use of DNA amplification by polymerase chain reaction (peR) for detection of Mycobacterium tuberculosis in 300 patients who were suspected of having pulmonary tuberculosis and compared the results with culture results which were performed in parallel with PCR. Two-thirds of each sample was processed for smear and culture by standard methods and one-third was prepared fo...
متن کاملReal time PCR for the rapid identification and drug susceptibility of Mycobacteria present in Bronchial washings
BACKGROUND Mycobacteria have a spectrum of virulence and different susceptibilities to antibiotics. Distinguishing mycobacterial species is vital as patients with non-tuberculous mycobacterial (NTM) infections present clinical features that are similar to those of patients with tuberculosis. Thus, rapid differentiation of Mycobacterium tuberculosis complex from NTM is critical to administer app...
متن کاملPerformance of an in-house real-time polymerase chain reaction for identification of Mycobacterium tuberculosis isolates in laboratory routine diagnosis from a high burden setting
Brazil is one of the high burden countries for tuberculosis, and a rapid diagnosis is essential for effective control of the disease. In the present study, an in-house real-time polymerase chain reaction (PCR) assay targeting the mpt64 gene for identification of Mycobacterium tuberculosis complex isolates was evaluated under routine diagnosis conditions in a reference laboratory. From May 2011 ...
